You can fill out the pdf on line and send it by selecting “Share” in the upper right hand corner of the page. Next select “Attach to Email” and then “Send Copy”. Address it to _CLOAKING If you download "Software Report.doc" you can fill it out, attach it to an email and send it to macbus.
